Text For Citation: 01 Item/Group: 001 Hazard: FALLING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) The hazard of falling, on or about January 30, 2000, and at times prior thereto, into an improperly guarded freight/passenger elevator shaft that did not meet the requirements of the American National Standards Institute Safety Code for Elevators (ANSI A17.1-1965): 1) The hoistway was not fully enclosed with a fire resistive enclosure (ref; 100.1a); 2) The gates on each platform level were not equipped with electrical interlocks (ref: 111.1b); 3) Each platform level gates mechanical interlocks did not operate as required (ref: 111.3b); 4) The gates did not protect the full height and width of the opening (ref: 204.6b); 5) The elevator car was not equipped with any type of car safety stopping device(s) (ref: 205.1) Among other methods, one feasible and acceptable abatement method to correct these hazards would be to renovate and upgrade this elevator to the requirements as stated in ANSI A17.1- 1965. --- Abatement certification or documentation is NOT required for this item. --- Abatement certification is required for this item. -X- Abatement certification and documentation are required for this item. --- Warning tag ( or citation copy) required on this equipment before moving.
