Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees due to being struck by flying objects: (a) Boiler room tunnel area; On or about December 27, 1999, employees installed a "Centerline" 16 inch gate valve with 7/8 inch bolts rather than the required 1 inch bolts. As the waterpipe was pressurized, the valve was blown away from the flange striking the worker. Among other methods, one feasible and acceptable method to correct the hazard is to establish protocol for the installation and inspections of piping systems in accordance with the manufacturer's specifications and the American National Standards Institute ANSI B31.1.
