Text For Citation: 01 Item/Group: 001 Hazard: CONFINED
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to toxic vapors while working in a confined space: Location:20 Old Mamoroneck Rd. White Plains, NY Leak containment vault, South end of the below-ground garage. On or about 9/19/ 2000 two employees from the Cardinal Tank Corporation were exposed to hazardous vapors of VM & P Naptha generated while painting an 11,000 gal oil storage tank housed in a leak containment vault. Abatement Notes: Among other methods, one feasible and acceptable abatement method to correct this hazard is to: Develop, implement and follow a confined space entry procedure as described in "The Guide to Confined Spaces" contained in the employers safety manual or as described in the employers' "Confined Space Awareness" training program. The procedures should include, but are not be limited to, the following items: 1.Conducting a confined space survey which identifies confined spaces. 2.Testing the air quality to determine adequate oxygen level and the presence of flammable and/or toxic gas/vapors. 3. Mechanically ventilating the space to remove air contaminants. 4.Training employees in confined space entry, testing, and use of personal protective equipment and respirators. 5. Stationing a standby person(s) outside the vault for communication and visual monitoring 6. Providing for emergency rescue operations.
