Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being caught-in, struck-by, and crushed by the mechanical components of a robotics system known as "the Alvey" machine: (a)Blueshift Industrial, Inc. at 800 N. 8th Ave. (Alvey Room, ConAgra Beef Plant) Greeley, CO 80631: On or about 04-04-00 and at times prior there to, the employer did not ensure that the path of travel of the Inserter/Extractor (I/E) car, a robotic component of the Alvey machine was guarded. Condition exposed employees to moving parts while employees installed platforms. (b)The Alvey Room: On or about 04-04-00 and at times prior there to, the employer did not ensure that the inbound path of travel of the conveyor system which fed into the Inserter/Extractor (I/E) car was guarded. Condition exposed employees to a caught-in and struck-by hazard. Among other methods, one feasible and acceptable abatement method to correct this hazard is to perform the following: ANSI/RIA R15.06-1999, American National Standard for Industrial Robots and Robot Systems--Safety Requirements, paragraphs 7-11; paragraph 11 Safeguarding devices- application requirements: "Safeguarding devices shall be designed, constructed, attached and maintained to ensure that personnel can not reach over, under, around, or through the device undetected and reach the hazard."
