Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries of the mast and derrick due to an improperly stabilized/guyed well servicing rig: a) The employer had not sufficiently safeguarded the stability of the 1981 Franks 300 well servicing rig by installing the guying system per the manufacturers recommendations. On June 25, 1999, while employees were removing tubing rods from Weld county Sec 20-1N-67 W, the well servicing rig collapsed/fell over fatally injuring a worker. Abatement Note: Abatement certification and documentation are required for this item (See enclosed "Sample Abatement-Certification Letter"). Abatement Note: In absence of mast manufacturer's recommendations or, where mast manufacturer's recommendations cannot be utilized because of obstructions at the wellsite location such as roads, pits, electrical lines, bogs, etc., the American Petroleum Institute publication RP 54 and 4G can be used.cation
