Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by flying objects. a) Giddings & Lewis 60" Vertical Turning Center #1784, Serial #5130054 78/96 - The auxiliary jaws of the machine were not adequately secured to the table and were ejected while the machine was being operated. On or about 12/13/99. NOTE:ABATEMENT DOCUMENTATION REQUIRED FOR THIS ITEM. See page 2,"Notification of Corrective Action" for instructions. NOTE: One feasible abatement method would be to follow manufacturer's recommendations and ensure the use of suitable work holding devices. Avoid excessive feeds and speeds. Make sure work holding devices are in good working condition. Never start machine unless you are absolutely certain the setup will hold parts. Require that all machinery components be rated for the way they will be used. This could be accomplished by having all components evaluated by a professional engineer.
