Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to:Falling from an elevation of approximately 12 feet. (a) West side of the building,employee was observed riding the forks of a rough terrain forklift truck(LULL) Highlander model 884c, that was elevated approximately 12 feet in the air.Fall protection was not provided. On or about 06/27/00 Abatement Notes: Among others,one suitable method for abating this hazardous condition is 1. Utilize only platforms designed for use elevating personnel with forklifts. These platforms must meet the requirements set forth in ANSI/ASME B56.6-1987 such as,but not limited to,fall protection for employees. 2. Train all employees about the hazards of falls associated with improper elevating methods,and the proper application and use of safety platforms.n
