Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck-by the falling loads: a)Route 11 & 81 bridge, on or about 6/5/01: Overhead chain hoist used for lifting work platforms: 1)4 hoists were not being inspected on a frequent and periodic basis. 2)Safety clip on hook was missing. ABATEMENT NOTES: Among other methods, one feasible method of abatement is to: 1). Conduct frequent and periodic inspections of the chain hoists for wear, deterioration and malfunction as per ASME B30.16-1987. 2). Replace safety clip on hook. Abatement certification must be submitted for this item.
