Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF BEING STRUCK-BY FALLING LOADS: a)On or about 03/12/01: The following overhead hoists had structures that were not rated, one had damaged links on hoist chain, another had a broken safety latch, and none of the hoists were being inspected, including but not limited to: 1.Gut room, Dayton 1/2 ton overhead hoist had a broken safety clip. 2.Gut room, Dayton 1/2 ton overhead hoist had two links of the lifting chain nearest the hook worn by approximately 50 3.Waste removal room, support structure for overhead hoist was not load rated. 4.Kill floor, Gut room, Calve skinning room and Waste removal room hoists were not being inspected on frequent and periodic basis. Among other methods, one feasible and acceptable method of abatement to correct these hazards is to: 1.Replace damaged safety clip on hook and worn chain. 2.Load rate overhead support structure. 3.Conduct frequent and periodic inspections or hoist and support structures for wear, deterioration and malfunction as per ASME B30.16-1987. b)On or about 03/12/01: The following below the hook lifting devices were not load rated, and were not being inspected on a frequent and periodic basis. 1.Gut room, lifting basket was not being inspected. 2.Gut room, lifting basket had no rated load capacity markings. 3.Gut room, lifting device had holes in welds. 4.Waste removal room, lifting device was not being inspected. 5.Waste removal room, lifting device had no rated load capacity markings. Among other methods, one feasible and acceptable method of abatement to correct these hazards is to: 1.Conduct frequent and periodic inspections of lifting devices for wear, deterioration or malfunction in accordance with ASME B30.20. 2.Prior to initial use, all new, modified or repaired lifting devices shall be inspected by a designated person and an operational test performed to verify compliance. c)Kill floor, on or about 03/15/01: The monorail transport system used to move cattle was not inspected on a frequent and periodic basis and the maximum load capacity was not posted. Among other methods, one feasible and acceptable method of abatement to correct these hazards is to: 1.Conduct frequent and periodic inspections of monorail system as per ASME B30.11-1973 and post rated load capacities for each section of the monorail. Abatement certification must be submitted for these items.
