Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load should the overhead hoist, support structure or below the hook lifting devices fail: a)On or about 08/10/99, the following overhead lifting devices had no rated load capacity markings and no initial, frequent or periodic inspections were performed, including but not limited to: 1)Department 110, below the hook lifting device used to lift rolls of steel. 2)Department 109, below the hook lifting device used in the grind area on hoist 33. 3)Department 116, below the hook lifting device in the handslide area. 4)Department 303, below the hook lifting devices in the clutch grind area on crane #7. 5)Department 111, below the hook lifting devices used to move reels, bundles, and baskets of chain. 6)Department 109, below the hook lifting device in the downstairs table area. ABATEMENT NOTES: Among other methods, one feasible and acceptable method to bate this hazard is to: 1)Prior to initial use, all new, modified, or repaired lifting devices shall be inspected by a designated person and an operational test performed to verify compliance. 2)Mark the rated load capacity of the lifting device on the main structure or equivalent. 3)Conduct frequent and periodic inspections of the lifting device for wear, deterioration, or malfunction in accordance with ASME B30.20. Abatement certification must be submitted for these items. b)On or about 08/10/99, thoughout the facility rated load tests were not completed for overhead hoists and underhung cranes. ABATEMENT NOTES: Among other methods, the following are feasible and acceptable abatement methods to correct each of these hazards are to: 1)Provide documentation from the manufacturer that rated load tests were completed for all overhead hoists. 2)Conduct load tests for overhead hoists in accordance with ASME B30.16-1987, and for underhung cranes in accordance with ASME B30.11-1973. Abatement certification must be submitted for this item.
