Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard(s) of being struck-by collapsing structural steel members due to the inadequate installation of replacement anchor bolts after it was found that the original bolts had been placed in the wrong pattern:One method of abatement to correct this apparent hazard is to request a field-modification through the project structural engineer of record.In addition the employer shall ensure that employees performing the repair work are adequately trained in the work changes and are fully supervised during the completion of all repair work.
