Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The spooling of the excess chain on the wagon was not monitored to prevent it from falling out and causing an injury to an employee operating and working around the J.D.Neuhaus Hebezeuge air hoist, model #B612, rated capacity 12 tons, downrated to 6 tons,at the Unit II boiler. The excess chain and stop block was not secured or contained to prevent the chain from falling. The air hoist was used to lift a can pump approximately 8-10 feet long and 2-3 feet in diameter and weighing approximately 10,900 lbs. from the ground level to the 4th level. The excess chain and stop block fell out of the wagon and struck the hoist operator at the 3 1/2 elevation level causing a skull fracture. Feasible abatement methods can be used to monitor,secure, or contain the excess chain.
