Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Employees using a 3/8" wire cable with one "crosby clamp" instead of the required two. Employees are exposed to the hazard of being struck by a 750 pound scaffold. a) Reference material on how to apply wire rope clips in the Handbook For Riggers page 54. b) Reference material found in ANSI B30.9a-1997- (9-2.7.4.1)page 32-33. Industry Practice: Use two clamps when using a 3/8" wire rope. Feasible Abatement: Put two clamps on wire rope.
