Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to pinch points/struck by hazards of mobile equipment: a) Machikas Construction: a DM-6865 Mack pumper truck in tow was not properly chocked, blocked and/or secured when an employee went between the trucks to reattach a tow chain, which resulted in the truck rolling and crushing the employee. The employer did not implement alternative safe work measures for his employees involved in the towing operation of coupling and uncoupling equipment being towed. Abatement Note: A feasible and useful abatement method to prevent this hazard from recurring would be to adopt safe practices to prevent accidents during towing, especially when coupling and uncoupling motorized equipment, as per the Accident prevention manual for business & Industry: Engineering & Technology, 10th Edition, Chapter 12, Haulage and Off-Road Equipment, Towing (under General Requirements) copyrighted 1992 by the National Safety Council. NOTE: ABATEMENT CERTIFICATION REQUIRED
