Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of overhead hoist collapse or failure : (a)Glue Line Area - The 1 ton Jet Hoist (Serial # 9510193/Model # 3-10/Plant # 43) the top hook had been stretched and the hoist had not been inspected since August 12, 1999. On August 12, 1999 the hook had been replaced since it was stretched as well. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS INSPECT HOIST IN ACCORDANCE WITH THE CURRENT AMERICAN SOCIETY OF MECHANICAL ENGINEERS/AMERICAN NATIONAL STANDARD INSTITUTE (ASME/ANSI) B30.16 - FOR OVERHEAD HOISTS. ASME/ANSI B30.16 FOR HEAVY SERVICE RECOMMENDS A VISUAL WEEKLY INSPECTION FOR THE FOLLOWING ITEMS: 1.ALL FUNCTIONAL OPERATING MECHANISMS FOR MALADJUSTMENT AND UNUSUAL SOUNDS; 2.LIMIT DEVICES FOR OPERATION; 3.HOOKS IN ACCORDANCE WITH ASME/ANSI B30.10; 4.HOOK LATCH OPERATION; AND 5.LOAD CHAIN. Note: Abatement certification required for this item applies to the weekly inspections. The hook in questioned was immediately replaced.
