Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: 23600 AURORA ROAD, BEDFORD HEIGHTS, OHIO: a.Tuesday, February 11, 2000, an employee, who was attaching an access ladder to the antenna platform, was exposed to the hazard of falling approximately 6-feet, 4-inches to the lower level. The employee was working on top of the platform while it was being suspended from a truck-mounted crane. b.Tuesday, February 11, 2000, two employees, who were tightening bolts on the antenna mounts, were exposed to the hazards of being struck-by the antenna platform. The employees were working beneath the platform while it was being suspended by a truck- mounted crane. Among other methods, one feasible and acceptable means to correct the hazardous condition may include: Block or support the antenna platform while the employees are attaching the ladder and tightening the bolts.
