Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of a defective weld repair made to a crack on the bridge girder of the C3 underhung crane. a)A crack on the bridge girder of the C3 underhung crane was identified by the crane inspections conducted at the plant. On or about November 28, 1998 a weld was made to the crack, but the weld was not evaluated for its weight carrying ability or structural integrity before the C3 crane was put back into service. The C3 crane was operated for two days until an inspection by qualified personnel revealed that the attempted repair had been defective. Among others, one feasible and acceptable method to abate this hazard is to have structural repairs and crane modifications checked by a qualified engineer or equipment manufacturer prior to placing the equipment back into service. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).s
