Text For Citation: 01 Item/Group: 001 Hazard: LOCKOUT
SCRR 71-112A: Failed to furnish a place of employment which is free of recognized hazards which may cause death or serious physical harm to his employees and comply with this regulation and other occupational safety and health rules and regulations promulgated under Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as amended, as follows: A. Employer knew of should have known that employee operating concrete pumping truck Nol. 39 at the jobsite was exposed to the hazard of electrocution, or being struck by falling debris from objects being hit due to loss of control of the boom of truck dispatched from the establishment and used on or about August 25, 2000 following report of a defective boom remote control box system and a defective boom hydraulic system on or about August 23, 2000. A feasible and useful method to correct this hazard, among other methods, is to develop and implement a machine/vehicle specific written lockout/tagout program that would identify, track and secure a truck with safety defects and also ensure all related personnel are informed that the vehicle has been locked out/tagged out to prevent use until repairs have been made.
