Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Hazard of being struck by high pressure water and pieces of the Klinger C-4401 gasket material: In the hydrostatic test area a gasket on a heat exchanger which was being hydrostatically tested was not fastened to the flange in such a manner as to prevent the gasket from being destroyed while undergoing hydrostatic testing. Feasible methods to correct the hazard include, but are not limited to, ensuring bolts are properly torqued and tightened in the appropriate sequence or using a gasket material designed to withstand the pressure at which the item is being tested. ***Certification and Documentation of Abatement Required***on
