Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On 06/15/99 at 1118 E. Main, Blanco, TX the employer failed to follow "Commentary and Recommendations for Handling, Installing and Bracing MetalPlate Connected Wood Trusses, HIB-91", thus exposing employees to a struck by hazard that could likely cause death of serious physical harm. Per Section 13.1 of HIB-91 "All temporary bracing (top chord, bottom chord and webs) should be no less than 2x4 grade marked lumber, 10 feet long and should have design values in accordance with the National Design specification for Wood Construction (NDS)". The employer used 1X4 grade wood as temporary lateral bracing. Per HIB-91 for a 42'- 60' scissors truss it is recommended that top chord lateral bracing be spaced/place every 5' (60 foot length truss / 5 foot lateral truss spacing= 12 top chord lateral trusses), the employer had 2 lateral top chord lateral trusses. Along with the above mention variation to the manufactures recommendations, other variations were also noted thus exacerbating the employee exposure to a hazard likely to cause death or serious physical harm. Had the employer followed the Truss Manufactures recommendations "HIB-91" this mishap would have be avoided.
