Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to the hazard of crushing injuries from slipping and/or falling spacer bar forklift attachment. a) Mid-South Yard Area Toyota Forklift - Model # T3000D - Serial # S-Z7-27735 Forklift was transporting two sections of precast concrete with an unsecured spacer bar attachment on or about 03/26/99. ONE FEASIBLE AND ACCEPTABLE METHOD OF ABATEMENT WOULD BE TO: (1) Follow the 1993 Edition American Society of Mechanical Engineers Standard B56.1-1993, Safety Standard for Low Lift and High Lift Trucks, Section 5.4.2., which states " When attachments are used , extra care shall be taken in securing, manipulating, positioning, and transporting the load." (2) Follow recognized industry practices for securing attachments to forklifts as found in brochures and operator's guides from manufacturers such as the Cascade Corporation and Trynex International.
