Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing or being struck by hazard: a) Main Floor: The employer did not require that employees driving skid steer loaders, such as, but not limited to the Bobcat Model 773, Serial 509646266, to look in the direction of the path of travel of the vehicle, such as but not limited to, traveling in reverse. Violation observed on or about 10/07/99. Feasible and acceptable methods, among others, to correct the hazard is to: a) Review the Driver's Manual. Follow the safety rule stating, "Look before backing up". b) Install a rear view mirror device for vehicles where turning around to look in the path of travel is not practical.
