Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a crushing hazard: a) Collection Routes: Employer failed to monitor employees operation of equipment, while on collection routes, to ensure that container latch arms are engaged to secure the container to the packer before dumping. Violation occurred on 4/7/99 "Among other methods, one feasible and acceptable abatement method to correct this hazard is 1) to develop and implement a program of periodically monitoring employees operation of equipment while on collection routes and taking appropriate action to ensure employees proper use of equipment, including adherence to safe practices in accordance with ANSI Z245.1, 1992 - Mobile Refuse Collection and Compaction Equipment Safety Requirements, Section 6.1.b" 2) Review with and distribute to drivers and lifters Leach Operators Manual and Leach Operator Safety Suggestions booklet. b) Garbage Trucks: Employer failed to ensure that container lifting hook safety latches were in good repair on garbage collection trucks such as but not limited to: Truck No. 640 and Truck No. 644. Violation observed on or about 4/14/99 "Among other methods, one feasible and acceptable abatement method to correct this hazard is 1) to develop and implement a program of inspection to ensure that hooks and latches are in good repair in accordance with ANSI Z245.1, 1992 -Mobile Refuse Collection and Compaction Equipment Safety Requirements, Section 7.2.2.6.1.b" 2) Perform daily and monthly inspections of the hook and latch as suggested in Leach Rear Loader Container Handling Systems manual, Section 4, Preventive Maintenance.
