KRS 338.031(1)(a): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: a.Sliding metal door at the north side of the Old Shipping Building was severely damaged and not properly secured to the track at the top, and presented a hazard to employees entering or exiting the building by way of the door opening in that they may be struck by the door in the event that it were to fall.
