Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Packaging Room - The "U" bolt rope clips attached to the wire rope of the Yale half ton electric chain hoist did not have the "U" bolt portion of the clips on the dead or short end of the rope. Employees were exposed to the hazard of being struck by a conveyor that was suspended with the use of improperly applied "U" bolt clips. b) Periodic and regular inspections of the cranes/hoists used throughout the facility were not conducted. Employees were exposed to the hazard of being struck by material and equipment that was being lifted by hoists that were used while in need of maintanance and/or repair. c) Packaging Room, Line #3 - The hook used with the Yale half ton electric chain hoist had wear beyond 15 percent of the normal throat opening. Employees were exposed to the hazard of being struck by the load in the event of hook failure. d) Packaging Room - The hooks used on the Yale half ton electric chain hoist of lines #1 and #3 were welded and placed into service without being load tested before being placed in service. Employees were exposed to the hazard of being struck by the load in the event of hook failure. e) Packaging Room - The side conveyors adjacent to the cookie regulating operation of line #1 and line #3 were suspended over employees by Yale half ton electric chain hoists. Employees were exposed to the hazard of being struck by the conveyors in the event of hoist failure. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d). Among feasible abatement methods is to comply with the ANSI standards for Overhead Hoists, ANSI B30.16 - 1973, and ANSI B30.10 -1987: 1. Attach the "U" bolts of "U" bolt rope clips to the dead or short end of the rope and remove wire rope that may have become damaged from improper installation of "U" bolt. 2. Conduct "frequent" and "periodic" inspections of the hoists. 3. Replace hooks that have wear exceeding 10 percent of original dimension. 4. Discard or test hooks that show defects or have been welded. 5. Avoid carrying loads over people.
