Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Bending Department: Press brakes equipped with a means for selecting "off" and "inch" for die setting, and one or more production modes of operation were not provided with a fixing of the selection which is controlled by the employer. The selecting switch was not provided with a key method, or other suitable means of selecting the mode of operation. Employees were exposed to the hazards of amputation and crushing injuries. Among other methods, one feasible and acceptable method to correct this hazard is to replace the switches with keyed switches that would provide employer control of the mode of operation (Reference: ANSI B11.3.) Abatement documentation is required for this item in accordance with 29 CFR 1903.19 paragraph (d).
