Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to; 1. Fragmentation and thermal injuries from cracking and ruptured autoclaves, and 2. Struck- by autoclave door while the autoclave is pressurized. a)Autoclave Bonding Area - Autoclaves in operation without the knowledge of the manufacturer's vessel and design specifications, especially the internal and maximum working pressures on or about 12/22/98. b)Autoclave Bonding Area - Autoclaves in operation without the development and performance of inspection procedures, inspection and testing of specific pressure vessel parts, controls and components, and maintenance of inspection and/or repair records, on or about 12/22/98. c)Autoclave Bonding Area - Autoclaves in operation without a safety door interlock to prevent accidental door operation while the autoclave is pressurized on or about 11/04/98. ONE FEASIBLE AND ACCEPTABLE METHOD OF ABATEMENT WOULD BE TO: 1. Follow the ASME Code, (American Society of Mechanical Engineers) Section VIII, Division 1 Boiler and Pressure Code Part UG 27, for pressure determination. 2. Follow the API 510 Code, (American Petroleum Institute) Pressure Vessel Code, Section 4.6 Records which mandates that records of manufacturer's data shall be maintained. 3. Follow the the API 510 Code, (American Petroleum Institute) Pressure Vessel Inspection Code, Section 4 Inspection of Pressure Vessels. 4. Follow the National Board Inspection Code Chapter 5 Inservice Inspection of Pressure Vessels by Authorized Owner-User Inspection Agencies. 5. Follow the ASME Code, (American Society of Mechanical Engineers) Section UG-35 for door closures. In addition follow the recommended manufacturing industry practice of installing door safety interlocks to prevent accidental door operation while the the autoclave is pressurized.
