Section 5(A)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Ice Manufacturing Plant, On or about January 5, 2000, employees were exposed to an "asphyxiation" hazard from pressure vessels containing anhydrous ammonia that was not examined, inspected or tested on a periodic basis to insure their mechanical integrity or continuing fitness for duty. Among other methods, one feasible and acceptable abatement method to correct this hazard is to insure that the company develops a vessel and piping inspection and test program complying with requirements in National Board Inspection Code (NBIC) - 1995, Sections RB-3200 "Inspection of Pressure Vessel," RB-3400 "Inspection of Piping Systems," RB-3700 "Inspection of Liquid Ammonia Vessels." PLEASE NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
