Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(A)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a)On or about 11/09/99, an untrained employee operating a forklift truck at the farm office area dismounted the forklift without lowering the forks, placing the transmission in neutral, and setting the brake to prevent movement of the forklift. The employee was subsequently crushed between the forklift and a flatbed truck when the forklift moved forward. Among other methods, a feasible method of abatement is to provide training in the safe operation of forklifts in accordance with the American Society of Mechanical Engineers' national consensus standard "Safety Standard for Low Lift and High Lift Trucks" (ASME B56.1-1993, 4.19) for all workers expected to operate fork lift trucks, including migrant workers.
