Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING

Title 40.1-51.1(a), Code of Virginia: The employer did not furnish employment and a place of employment which were free of recognized hazards that were likely to cause death or serious physical harm to employee in that : (a) Located at Industrial Resources Technologies, Inc., employees were exposed to crushing hazards while operating forklifts on a service road where the forklifts were subject to tipping due to the presence of rough terrain. On July 30, 1998 employees using forklifts were loading a tractor trailer with spent toner cartridges along a service road which had a 3 to 5 inch ridge along the north edge of the road. The Model GC20 Caterpillar forklift Serial Number 4EMO7439 was backing up when the left rear tire hit the edge of the pavement causing the forklift to tip over.
