Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards: a)Mammoth Cave Visitor's Center - On or about 04/07/11, employees were exposed to struck-by hazards when the outriggers were not set correctly causing the pump truck to tip over. Among other methods, one feasible and acceptable means of abatement would be to the follow the American Concrete Pumping Association (ACPA) Safety Bulletin: Setting Outriggers to Prevent Accidents which states, "Always use the factory supplied cribbing pads or more, no matter what the surface, even if it is on concrete." In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
