Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushing hazards: a)T-9145 Desert Storm, Ft. Campbell, KY - On or about 11/18/10, an employee was exposed to a struck-by/crushing hazard when not wearing a seat belt while operating a Pettibone rough terrain forklift. Among other methods, one feasible and acceptable method to correct this hazard would be to follow manufacturers's recommendation which states to wear seat belts at all times.
