Text For Citation: 01 Item/Group: 001 Hazard: LOCKOUT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of the inadvertent release of stored energy: (a)M/V Sea Alaska: Proper documentation of the periodic inspection/certification of energy control procedures was incomplete and did not contain the required documentation as to the identification of the machines/equipment, inspection date, name of affected employees and/or name of the employee performing the inspection. There was no written documentation to ensure that the procedures/requirements of 1910.147 are being followed. Among other methods, one feasible and acceptable abatement method to correct this hazard would be to develop and implement a written lockout/tagout program.
