Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1): of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were expose to: The hazard of falling from an elevated work platform. Location: Building 4 Winter Berry Lane On or about: 05/13/97 (a) Employee was working in an elevated position at the 2nd floor window 15 feet above the ground level of building # 4. The work platform, a wooden pallet approximately 4 feet wide by 10 feet long was not secured or fastened to the lifting carriage or forks of the Lull Highlander serial # 944 TT 34. Also the employee was not provided with fall protection system. AMONG OTHER METHODS ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS (1) Provide employees fall protection for all elevation and work location more than 6 feet above the ground level. (2)Follow the safety procedures in ANSI B56.1.1975 section 427 A to D and 513 A to B American National Standard for Low Lift and High Lift Trucks and / or ASME B56.61992 SECTION 5.15 A to Y. American Society of Mechanical Engineers Safety Standard for Rough Terrain Forklift Trucks. (3)Secure the work platform (wooden pallet) to the lifting carriage and / or forks before it is elevated. (4)Instruct and train all fork lift operators to observed the Safety Rules for Forklift operations.
