Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: The hazard of being struck by an exploding tire and rim assembly. Location: 194 Route 59, Suffern, NY 10901 On or about 06/27/96: a) Employee(s) that engage in the servicing of automotive and light truck tire / wheel assemblies, which include the mounting and demounting of tires onto and off rims, and related activities such as inflating, installing, and handling, are not provided with adequate training to recognize and prevent hazards that can lead to serious injury or death. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS..." 1) Read and follow the procedures of any tire manufacturer warning, contained in product literature or molded into the sidewall of a tire. 2) Follow the warning on page 1, of the operating instructions, provided by Hennessy Industries, Inc., for the Coats rim clamp tire changer - model #RC-15A (publication #181531-A). 3) Follow the procedures and safety precautions set forth throughout the operating instructions (publication #181531-A), provided by Hennessy Industries, Inc., for the Coats rim clamp tire changer - model #RC-15A. 4) Follow the Rubber Manufacturers Association (RMA) procedures for automotive and light truck tire demounting and mounting, contained in RMA publication titled "Demounting and mounting procedures for automobile and light truck (LT) tires that are used on single-piece rims" (publication #DMPAT-7/95).
