Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The Employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to tipping/falling/crushing hazards: a) On and/or about 4-28-09, Employees were observed using a "JLG Model-600S" Aerial manlift on an uneven/unstable surface, exposing them to tipping/falling/crushing hazards from elevations of 30-feet or more from the ground below, at the U.S. Army Human Resource Center (HRC); Ft. Knox, KY. This hazard was reasonably foreseeable in that it was recognized by employer representatives. Among other methods, one feasible and acceptable method to correct this hazard would be follow company policy, ANSI A-92.5 for workplace inspections, operation and during use, described in Ch.8, Items a) through g), and retrain employees on existing work rules for communication.
