Section 6(a) of the Puerto Rico Occupational Safety and Health Act: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or physical harm to employees by which employees were exposed to: Seccion 6(a) de la Ley de Seguridad y Salud en el Trabajo de Puerto Rico: El patrono no proveyo empleo y un sitio de empleo libre de riesgos reconocidos que esten causando o que puedan causar muerte o dano fisico a los empleados, por cuanto empleados estaban expuestos a: At the first aisle in the front and back area of the warehouse, there were two loose and unstable metal plates with approximately 64" length by 64 " wide and 1" 1/2" of depth, located in the holes on the floor. An employee was walking and another employee was driving with a forklift trademark Raymond, model 261-OPC 30, and serial number 261- 02-01812. They were storing and checking merchandise. Hazard: Falling load and toppled of the forklift. Among other correction methods, one feasible and acceptable to correct this hazard is to provide a fixed cover to level the floor. En el primer pasillo del almacen en la parte del frente y en la parte de atras, habian dos placas de metal sueltas e inestables con aproximadamente 64" de largo x 64" de ancho y 1" 1/2 " de profundidad, localizada sobre los huecos del piso. Un empleado estaba caminando y otro empleado conduciendo un montacargas marca Raymond, modelo 261- OPC 30, y numero de serie 261-02-01812. Ellos estaban almacenando y verificando mercancia. Riesgo: Caida de la carga y vuelco del montacargas. Entre otros metodos de correccion, uno factible y aceptable para corregir este riesgo es proveer una cubierta fija para nivelar.
