Text For Citation: 01 Item/Group: 001 Hazard: EXPLOSION
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an explosion/flash fire hazard: a) Mantua Grove Road, Woodbury, NJ 08096 - On or about 11/29/97, employees were vacuuming polypropylene in powder form from a storage silo on the property of Huntsman Polypropylene Corporation utilizing a Vactor Mobile Industrial Vacuum Loading Machine (manufactured by Peabody Meyers) without it being grounded and/or bonded. NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the sections of your publication titled, "Allstate Power-Vac Health and Safety Forms" that address vehicle grounding and bonding. ABATEMENT NOTE: In Accordance with 29 CFR 1903.19, Abatement Verification, the employer will furnish certification that the described hazard has been abated. Documentation may include but is not limited to photographs, videos, work orders, invoices, training records and consultant reports. Abatement documentation must be received by the Area Director within 13 calendar days of the final abatement date for this violation.
