Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries: a) Employees of Tagawa Greenhouses, Inc. were exposed to crushing injuries as a result of an unsecured tank and motorized mixing assembly in Area #10 becoming unbalanced. On 3/3/97, one employee was fatally injured when one 200-gallon tank and motorized mixing assembly fell onto the worker as the worker fell off of the tank-supporting platform. Among other methods, one feasible and acceptable abatement method to correct this hazard is to secure the motorized mixing assembly to the platform or wall with bolts and/or chains.
