Text For Citation: 01 Item/Group: 001 Hazard: FALLING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards from an elevated personnel platform: (a)Wyatt Construction Co., Inc. at 3900 Colorado Avenue, (Bldg. A), Boulder, CO: On or before 08-27-97, the controlling employer did not ensure that a wooden, job-made personnel platform used in conjunction with a rough-terrain forklift (Dynalift D4P 80) was properly designed, constructed, secured, utilized and maintained. Condition exposed employees to hazard of fall from elevation. Among other methods, one feasible and acceptable abatement method to correct this hazard is to design, construct, utilize and maintain the personnel platform in accordance with ASME/ANSI B56.6-1992, Safety Standard for Rough Terrain Forklift Trucks.
