Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load: In the Pigging area, on or about 5-10-99: Employees were exposed to the hazard of being struck by the load should the components of the overhead hoist fail on equipment including but not limited to: a)The #1 P & H 2 ton ladle hoist did not have a safety latch on the hook. b)The #3 Polaris 2 ton ladle hoist did not have a safety latch on the hook. c)The 1 ton Ingersol Rand pneumatic hoist # 9031 the hook was wrapped around the casting and then back on to the load chain. The hook also did not have a safety latch. d)The 1 ton Ingersol Rand pneumatic hoist RCB 7731 did not have a safety latch. Among other methods, several feasible and acceptable abatement methods to correct each of these hazards are to: a)Replace the safety latches on the hooks. b)Replace the safety latches on the hooks. c)Replace the safety latches and retrain employees on the correct way to operate the hoists. d)Replace the safety latches on the hooks. Abatement documentation must be submitted for this item.
