Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards: a)Titan Building, on or about 10/08/98: A Coffing 5 ton Electric Chain Hoist, model number EC-10008, used to move products, had the hook throat latch missing from the load hook. b)Small tank area, on or about 09/30/98: A Coffing 3 ton Electric Chain Hoist, model number EC-6016-7, used to move pieces of steel between the circular shear and the flanger, had the hook throat latch missing from the load hook. c)275 line, on or about 09/30/98: A Coffing 2 ton Electric Wire Rope Hoist, model number WR-4014-3, used to move product, small tanks on the monorail, had the hook throat latch missing from the load hook. d)275 line, on or about 09/30/98: A Yale 2 ton Electric Wire Rope Hoist, model number 421L, used to move small tanks, had the hook throat latch missing from the load hook. e)Lee's area, on or about 09/30/98: A C & M 2 ton Electric Chain Hoist, model R, used to move steel, had the hook throat latch missing from the load hook. f)64 inch line, on or about 09/30/98 through 11/25/98: Coffing 3 ton Electric Chain Hoist, model number EC-6010, used to move tanks, had the throat latch missing from the load hook. g)64 inch line, on or about 09/30/98: A C & M 2 ton Electric Chain Hoist, model R, used to move tanks, had the throat latch missing from the load hook. h)64 inch line, on or about 09/30/98 through 11/25/98: A Coffing 3 ton Electric Chain Hoist, model number EC-60107, used to move tanks, had the throat latch missing from the load hook. i)Spray painting area, north wall, on or about 09/30/98: A C & M 1/2 ton Electric Chain Hoist, model WF, used to move small metal pieces for painting, had the throat latch missing from the load hook. ABATEMENT NOTES: Among other methods, one feasible and acceptable abatement method to correct the hazard is to: 1.Install hook throat latches. 2.Follow the requirements of American National Standard for Air Chain Hoists (ANSI/ASME HST-5M-1985). j)East side of the Big tank area, on or about 09/30/98: A Demag 10 ton overhead hoist did not have clearly labeled pendant controls, exposing employees to the hazard of being struck by the load should the overhead hoist lifting device fail. ABATEMENT NOTES: Among other methods, one feasible and acceptable abatement method to correct this hazard is to: 1.Relabel pendant control device for identification of functions. 2.Conduct frequent/periodic inspections and perform the required maintenance following these inspections in accordance with ANSI B30.16,-1987 Chapter 16-2. Abatement documentation must be submitted for this item.
