Text For Citation: 01 Item/Group: 001 Hazard: LIFTING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load should the lifting device fail: On or about August 27, 1997, in the Process Area: The R&M Chester Hoist, Serial Number H29841 Type F2-BG, used to move motors, steel decking, and tanks: a)Had been modified by changing the motor and had not been inspected by or under the direction of a designated person prior to initial use. b)Had not received frequent inspections which included a daily examination of the braking mechanism; rope; hooks for deformation or cracks. Abatement methods include but are not limited to: 1.Conduct initial, frequent and periodic inspections of the lifting devices by designated personnel for wear, deterioration or malfunction per the ANSI B30.16. 2.Maintain complete written inspection records which are signed, dated and readily available to appointed personnel (reference ANSI B30.16-1973). Abatement documentation must be submitted for this Citation.
