Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load should the components of the overhead hoist fail: a)TR-3 Building, Screw Manufacturing Dept., Henry Filter Area, on or about 8/21/97, Electrolift overhead hoist #71, the wire rope was not properly seated on the drum in that it was overlapping on one side of the drum, there were no wraps of the rope remaining on the drum when the hook was lowered in its extreme lowest position and there was a kink in the wire rope, exposing employees to being struck by the load. Among other methods, several feasible and acceptable abatement methods to correct these hazards are to: 1.Install warning labels to hoist, load block, or controls concerning cautionary language against operating hoist when load is not centered under hoist; operating hoist with twisted, kinked, or damaged rope; operating a rope hoist with a rope that is not properly seated in its groove, etc. in accordance with ANSI B30.16-1987, Chapter 16-1.1.4. 2.Install a lower limit switch or insure that at least two wraps remain when the hook is in its lowest position in accordance with ANSI B30.16-1987, Chapter 16-1.2.6. 3.Insure employees are instructed in the proper operation of the hoist in accordance with ANSI B30.16-1987, Chapter 16-3.1.1. 4.Insure that any deficiencies found by the operator are reported to a designated person and any deficiencies found during annual and periodic inspections, that required repair work is performed. 5.Insure that wire rope is inspected in accordance with ANSI B30.16-1987, Chapter 16-2.4.
