Text For Citation: 01 Item/Group: 001 Hazard: LIFTING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load should the below-the-hook lifting devices fail: a)TR-3 BUILDING, DEPT. 338A, ON OR ABOUT 6/7/96, BELOW-THEHOOK LIFTING DEVICES USED FOR SUSPENDING COMPRESSORS IN A WHEELABRATOR FOR SHOTBLASTING OPERATION HAD NO RATED LOAD MARKINGS, NO OPERATIONAL TESTS PERFORMED AND NO INITIAL, FREQUENT OR PERIODIC INSPECTION PERFORMED BY DESIGNATED PERSONNEL. LIFTING DEVICES WERE EXPOSED TO BEING SHOTBLASTED AND IT WAS UNKNOWN WHAT THEY WERE RATED FOR AFTER EACH SHOTBLAST OPERATION. AMONG OTHER METHODS, SEVERAL FEASIBLE AND ACCEPTABLE ABATEMENT METHODS TO CORRECT THESE HAZARDS ARE TO: 1.MARK THE RATED LOAD OF THE LIFTING DEVICE ON THE MAIN STRUCTURE WHERE IT IS VISIBLE PER ASME B30.20-1993. 2.CONDUCT INITIAL, FREQUENT AND PERIODIC INSPECTIONS OF THE LIFTING DEVICES BY DESIGNATED PERSONNEL FOR WEAR, DETERIORATION OR MALFUNCTION PER ASME B30.20-1993. 3.DURING SHOTBLAST OPERATIONS WHERE THE LIFTING DEVICE IS EXPOSED, USE A PROTECTIVE COVER OVER THE DEVICE TO PREVENT DAMAGE FROM THE STEEL SHOT, OR 4.PRIOR TO INITIAL USE AND AFTER EACH SHOTBLAST OPERATION HAS OCCURRED IN THE WHEELABRATOR, EACH LIFTING DEVICE SHALL BE TESTED BY OR UNDER THE DIRECTION OF AN APPOINTED PERSON AND A WRITTEN REPORT FURNISHED BY SUCH PERSON CONFIRMING THE LOAD RATING OF THE DEVICE PER ASME B30.20-1993.,
