Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by tipping or falling machinery: a)On or before July 13, 1998, on the roof, employees who installed a powered platform for building maintenance on its track did not lock the removable track section in place after installing the unit. ABATEMENT NOTE: Among others, a feasible method to eliminate or to materially reduce this hazard is to comply with the "Operating Instructions for Manually-Propelled, Rolling Dolly, Avery Fisher Hall - Job #1416" and lock the removable track section in place. b)On or before July 13, 1998, on the roof, employees who moved a manually propelled powered platform for building maintenance along its track to the work location did not attach the draw bar from the counterweight dolly to the unit and move the assembly together to the work location. ABATEMENT NOTE: Among others, a feasible method to eliminate or to materially reduce this hazard is comply with the "Operating Instructions for Manually-Propelled Rolling Dolly, Avery Fisher hall - Job # 1416" and attach the counterweight dolly to the unit and move the assembly together to the work location.
