Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of crushing injuries: On or about 7-5-2000,for the one person electric elevator (also known as a manlift) located near the grain dump, there was no type of guarding at the bottom elevator landing area to prevent an employee from being under the elevator as it descended. The bin floor elevator landing and the top floor elevator landing were also inadequately guarded to prevent an employee from falling into the evelator shaft when the elevator was not at that particular landing area. These conditions exposed the employee(s) to the hazards of being crushed-by or struck-by the descending elevator or falling into the elevator shaft. One method of abatement to correct this hazard is to guard the full height and width of the elevator openings as specified in the American National Standard Safety Code ANSI A17.1- 1993, Section 110, Rule 110.1. ***THIS ITEM REQUIRES A WRITTEN CERTIFICATION OF ABATEMENT***
