Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being pinned under, crushed, or thrown from a forklift truck: a)On or about 07/31/98 - warehouse area Employee was operating a Yale Forklift truck, serial number A808V04374T, without wearing the provided seatbelt. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD IS TO: Require and effectively enforce the use of operator restraint systems provided when operating forklift trucks, as referenced in the Yale operators manual and ANSI/ASME B56.1-1993, Safety Standard for Low Lift and High Lift Trucks, Section 5.3.19 Operating Safety Rules and Practices. ABATEMENT VERIFICATION IS NOT REQUIRED.ION
