Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: UTI Drilling Company Lonnie Ray B #4 lease Midland County, Texas For the time period up to including May 27, 2000. The employer through the Tool Pusher or other designated representives did not assure that all employees had attended the required training programs for new hire employees prior to allowing the employee to work on the rig. On May 27, 2000 an employee who had not been trained on the Rig Pass Program was allowed to climb the ladder utilizing the climb assist to perform work activities in association with the rigging down process.. API RP 54 5.1.3 addresses the indoctrination of new employees regardless of past experience.
