Text For Citation: 01 Item/Group: 001 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: the hazards of severe injuries and or death associated with exposure to fall hazards of 30 feet high. a) 950 S. Central McKinney Texas. Two employees of Warnick Construction were elevated on a workplatform suspended on the forks of a Sky Trak Rough Terrain Forklift. The following hazards were observed: 1. One of the employees was observed not tied off or secured to the workplatform. A danger of falling 12-15 feet was present. 2. One employee on the platform suspended on the Sky Trak forklift was observed climbing onto the top of the suspended platforms top guardrail. Employee was sitting on top of the top rail with out fall protection, he was not tied off. Both employees were wearing a body harness and lanyards (beamers). A danger of falling 12-15 feet was present. 3. The Sky Trak Operator left the Sky Trak unattended while two employees were suspended/elevated up approximately 12-15 feet high. The operator walked to the south of the Sky Trak approximately 41 feet away. One feasible method of correcting this hazard among others is to implement and follow ANSI/ASME B56.6 - 1992 Section 5.15; Elevating Personnel: 5.15.1(k): Insure that required restraint means such as railings,chains,body belts with lanyards are in place and properly used. 5.15.1(s): A trained operator shall be in position to control the rough terrain forklift. (see 6.2.11 below) 5.15.1(w): Platform shall be lowered to the floor level for personnel to enter and exit. Personnel shall not climb on any part of the rough terrain forklift truck in attempting to enter or exit. 6.2.11: A rough Terrain Forklift Truck is unattended when the operator is 25 feet or more from the truck.
